Parallel to Program::functions, because a 1:1 encoding keeps a
function’s program counters exactly as they were: code[id][pc] is the
encoding of program.functions[id].code[pc], and everything indexed by pc
— spans, local ranges, switch targets — goes on meaning what it meant.
